Transferring to Another C&MA Employer

C&MA benefit plans give you increased flexibility to transfer between many C&MA employers nationwide. Learn how to transition seamlessly:

 

Transferring Health Coverage

  • You may transfer your health benefits between participating employers.
  • Use Enrollment and Change Form; Employer Certification Form; Employer Bank Authorization Form and mail or fax to Alliance Benefits.
  • If you remain in the same medical plan (for example, Plan A Premium), your deductible will not start over in that calendar year (even if you are on Coverage Extension) as long as there is no gap in coverage.
  • If your hire date is the first of the month, coverage starts that day. Otherwise, coverage starts on the first of following month.
  • If your new employment will not start immediately, you may be able to purchase Coverage Extension to cover the gap.
