Healthy Rewards

As part of the C&MA's commitment toward providing you with the tools and resources to lead a healthier lifestyle, take the first step to better health and earn an incentive for doing so. You and your spouse are invited to take Highmark's online Wellness Profile, which will give you a "snapshot" of your overall health. You'll find out positive aspects of your health and lifestylye as well as areas where you may need some improvement.

 

More about the Visa Gift Card from Alliance Benefits*

  • $100 Visa Gift Card when you take Highmark's Wellness Profile (effective January 1, 2011)
  • You and your spouse may each receive rewards annually if there is a nine month gap between completion
  • Additional $50 Visa Gift Card when you complete the Profile and also complete the Wellness Profile and also complete a physical exam accourding to the set guidelines in the Preventative Schedule (effective January 1, 2011)
  • You may spend your gift card whereever Visa is accepted. Or if you wish to donate your gift(s) to the Great Commission Fund*, you may call the C&MA Office of Donor Accounting at (866) 443-8262

 

More about the Highmark Wellness Profile

  • It is through Highmark and Confidential. Alliance Benefits does not have access to the information entered on this on-line survey. We are given only a summary report so we can make improvements to the plan
  • It is most helpful to take the profile after your annual check-up so you can enter current health numbers such as blood pressure or cholesterol levels
  • Allow 30 minutes to complete the profile
  • The questionnaire will not mention the $100 reward but you will receive it automatically if eligible

 

To take the Wellness Profile

  • Log in to www.highmarkbcbs.com
  • Under the tab "My Health" , select Wellness Profile, also called "Health Media Succeed Questionaire"

 

*$100 and $50 rewards are financed solely by Alliance Benefits and do not come out of the Great Commission Fund.
