- When you enroll in any of our C&MA Health Plans, you are automatically enrolled in Basic Life insurance and you have the opportunity to purchase Voluntary Life Insurance
- If you are enrolling in a Health Plan, you may purchase Voluntary Life using the same Health Enrollment form
- If you are already enrolled in the Health Plan and now want to add or increase Voluntary Life Insurance at a later time, please see Claims and Changes
- Life Insurance is not available for purchase separately from Health Plan enrollment.
