Claims Incurred Out of the US

When you obtain medical services out of the US, claim submission is very streamlined.

Submit Claims to:

Please Note:  Prescriptions purchased out of US are reimbursed through Meritain, not Express Scripts.

 

To submit claims, e-mail this information to Alliance@meritain.com for each claim:

  • Primary Cardholder’s Name
  • Meritain ID number
  • Patient’s Name
  • Patient’s Date of Birth
  • Date of Procedure
  • Diagnosis (Medical condition or symptoms)
  • Type of Procedure (or name of medicine)
  • Amount of bill for each procedure in local currency
  • Exchange rate on date of service
  • Amount of bill for each procedure in US dollars according to the exchange rate on date of service

 

If the claim is US $2,000 or more: You must attach a copy of itemized receipt.

 

If the bill or receipt is in another language: Please provide an English translation.

 

For claims less than US $2,000: You do not need to submit receipts, but you should retain all receipts for at least two years from date of service in case of audit.

 

Claims must be filed within one year from date of service.

  • Save your claim in your Sent e-mail folder until paid
  • Check your claims at www.myMeritain.com to verify Meritain has processed
  • If your paid claim online is labeled NOCHECK, reimbursement will appear in the next available month’s allowance

  • Please save all medical receipts, travel receipts, and doctor’s notes.
  • Keep record of any work time lost.
