Because we know how important good eye care is to overall health, Alliance Benefits participants enrolled in any medical plan with the Premium Plan option, receive vision coverage through a Superior Vision PPO Plan. The Superior Vision plan has a network of over 35,000 providers, including more that 5,000 ophthalmologists.

Through your Superior Vison PPO Plan:

• You may choose any vision provider, but you will experience the greatest savings if you use a Superior Vision PPO provider
• You may use a non-participating doctor or provider, but your reimbursement is limited to amounts Superior Vision has set
• You may find a participating vision provider at www.superiorvision.com
• Find discount retailers, see the eyewear store listed separately from the exam provider. Please confirm both before scheduling

Vision coverage is a bundled product and is not available for enrollment apart from a medical plan with the premium option.
