Leave of Absence
When you are no longer actively working 20 hours per week, health benefits must end.
- If you go on Leave of Absence*, this is viewed just like employment ending.
- Your employer must notify Alliance Benefits within 30 days, using the Form to End Active Coverage.
- Your Health Plan will end on the last day of the last month in which you actively work 20+ hours per week. Coverage Extension may be available within 30 days.
- Your Life Insurance and Long Term Disability will end on the last day of the month in which you actively work 20+ hours per week. Portability or Conversion may be available within 30 days.
- Your 403(b) contributions must end when you are no longer a C&MA employee. If you later begin new C&MA employment, you may resume contributions to your 403(b) account.
*If you go on Medical Leave of Absence, please notify Alliance Benefits promptly. You may be able to file a Long Term Disability claim.
