  • Generally if you inform Alliance Benefits within 30 days of a job or family status change you will not be required to take a medical history and/or paramedical exam
  • Increases outside of health plan enrollment, employment changes, and family status changes, are likely to require medical history and/or paramedical exam
  • Please use the Enrollment and Change Form

 

End or Decrease Voluntary Life
  • Please send written notice to Alliance Benefits
    • Specify the month you want coverage to end
    • Allow two weeks to process change
  • If you change your mind or want to re-enroll, enrollment will be subject to approval
